- Name every business owner and the ownership structure on the application.
- Meet the health, safety and equipment standards in 16 TAC Chapter 83.
- No living or sleeping on the licensed premises; a residence-attached salon needs a separate entrance with the connecting door closed in business hours.
- All requirements within one year of receipt or the application is void.
